I became interested in pathology, and specifically pediatric pathology, in medical school after being involved in a research project that explored transcription factors in retinoblastoma with Dr. David Eisenstat in Manitoba, and a subsequent ocular pathology medical elective with Dr. Miguel Burnier Jr in Montreal. Once in my anatomical pathology residency, I continued to pursue pediatric pathology but also took an interest in leadership, administration, and quality improvement. This led me to Chicago where I worked under a fantastic group of mentors including Pauline Chou, Mike Fritsch, Hector Melin, and Linda Ernst. I was lucky to find a job in my wife’s hometown of Vancouver, where I practice general pediatric and perinatal pathology with particular clinical and research interests in pediatric tumors including osteosarcoma, gastrointestinal diseases including eosinophilic disorders, and developmental and perinatal pathology including congenital heart anomalies. Bear Down! Go Cubbies!

Research Interest

My research interests are diverse but focused. I have a firm interest in bone tumors and a soft spot for soft tissue sarcomas, including understanding the underlying pathophysiology and translational work in the clinical pathology lab setting. I joined the 2019-2021 cohort within the Children’s Oncology Group Young Investigator Program with a focus in the bone tumor group. I also am interested in exploring cell signaling and cell-to-cell communication through conventional molecular and proteomic analysis with fellow investigators. My second area of research interest is in gastrointestinal pathology, particularly eosinophilic esophagitis (EoE). Through these EoE projects, I enjoyed expanding my experience in immunology which builds on an interest during my fellowship at Lurie Children’s Hospital of Chicago, which explored transplant immunology and the effect on the gastrointestinal tract. I also enjoy expanding my experience in the area of transplant immunology. My last primary area of interest is in developmental pathology, which often stems from identifying common pathologic findings in the interesting cases I encounter at BC Children’s and Women’s Hospital. I have an interest in leadership and administration, which blossomed during my time as Cohort 2 in the BC Patient Safety and Quality Council’s Clinician Quality Academy.

Current Projects In My Lab Include

  • Pediatric Bone Tumors The Bush lab uses fresh and fixed tumor tissue from the two most common malignant bone tumors, Osteosarcoma (OS) and Ewing Sarcoma (ES), to explore novel antibodies using immunohistochemistry (IHC) for diagnosis and prognosis. This includes developing tissue microarrays (TMAs) to test a number of antibodies across a wide variety of samples. Some work has shown that a particular methylation pattern in the diagnostic biopsies of OS may predict good or bad disease behaviour.
  • SWI/SNF and Rhabdoid Tumors (RTs) Rhabdoid tumors are a group of primitive cancers that are often highly aggressive and are generally defined by a mutation in the SWI/SNF pathway, including the genes SMARCB1 (INI1) and SMARCA4 (BRG1). The Bush lab was one of the first groups to explore the use of BRG1 IHC to identify a subset of RTs that were INI1-retained. Further work is ongoing looking at the immune microenvironment of RTs as these are generally considered to have low tumor mutational burden and thus are immune-cold tumors, yet there appears to be heterogenous areas of tumor cell infiltration. Utilizing spatial genomics and proteomics, we hope to better understand RT inter and intra-tumoral heterogeneity.
  • Novel Diagnostic Techniques in Pediatric Tumors Working with collaborators, the Bush lab is helping to establish a proteomics pipeline that will demonstrate the utility of proteomics in clinical applications. This includes providing diagnostic, prognostic, and in some cases predictive markers that will allow for personalized therapies based on relative over or under-abundance of particular proteins. Promising results have been seen in OS and neuroblastoma to date. In collaboration with researchers at UBC and the NIH, the Bush lab is exploring the use of machine learning (ML) in osteosarcoma. This work will develop a pipeline for assessing tissue quality for the purpose of molecular studies, facilitate histologic classification of OS for clinical trials, and to explore potential prognostic markers as identified by supervised and unsupervised ML.
  • Precision Oncology The Bush lab has worked with collaborators in establishing a novel method of developing patient-derived xenografts using a less expensive model than the typical mouse system. Through this chorioallantoic membrane (CAM) modelling system, we have been able to facilitate tumor expansion in both highly aggressive and indolent tumors, and have worked to demonstrate the utility of the CAM system for drug testing.
  • Eosinophilic Esophagitis (EoE) Our group was the first to show that pediatric EoE was associated with increased lamina propria IgG4-positive plasma cells, which is being explored as to whether food-specific IgG4 can be used to evaluate for EoE disease activity. Ongoing work by the Bush lab has included utilizing novel techniques to evaluate EoE slides as well as potential spatial differences between the immune and squamous cell compartments.
  • Perinatal, Placental, and Autopsy Pathology The Bush lab continues to have an interest in a number of additional projects, including the pathology seen in placenta, stillbirth pathology, and family/caregiver experiences in sudden death in infancy and childhood.

  • Infection prevention and control
  • Hand hygiene
  • Behaviour change
  • Organizational culture

The field of infection prevention and control aims to prevent patients from getting infections in the hospital. We use interventions such as hand hygiene, isolation precautions, and environmental cleaning in order to accomplish this. In the past, much of our work has involved developing policies and educating healthcare workers, but we know that people do not always comply and infections continue to occur. My research focuses on novel ways to change the behaviour of healthcare workers and patients and to change the culture of an organization in order to achieve the goal of reducing infections.

I am also interested in new technologies to monitor hand hygiene compliance. Currently we determine if healthcare workers are cleaning their hands appropriately by watching them, but this is not always accurate. Systems that monitor hand hygiene using technologies such as real-time locating systems and video monitoring may be more accurate and can potentially be used as tools for improvement. I am interested in carrying out evaluations of these systems to confirm their utility.

Current Projects In My Lab Include

  • Predicting hand hygiene behavior among health care workers based on implicit and explicit attitudes.
  • Quality improvement projects to improve patient and visitor hand hygiene in hospital.
  • Pilot project to evaluate the use of an electronic hand hygiene monitoring system in a long-term care facility.

Dr. Michael Sutherland received his PhD from the University of Ottawa, Department of Biochemistry, Microbiology and Immunology in 2003. Since then, Michael has been working with Dr Ed Pryzdial as a Research Associate with Canadian Blood Services at the UBC Center for Blood Research. The focus of Michael’s research is the modulation of haemostasis by two enveloped viruses; herpes simplex type 1 (HSV1) and dengue virus (DENV). The use of HSV1 has lead to identification of novel pathways to initiate coagulation directly on the virus surface. This activation is facilitated by both normal blood proteins and proteins encoded by the virus genome. Ultimately, these pathways may enhance the susceptibility of host cells to infection and contribute to the risk of vascular pathology. Studies with DENV have revealed that DENV can bind and infect platelets with the eventual production of infectious DENV progeny. This provides a possible additional mechanism for the thrombocytopenia that is often seen clinically with DENV infection. Identification of DENV replication within blood cells also have broader implications of other clinically important RNA viruses.

In addition to many scientific endeavours, Dr. Sutherland is involved in medical education and currently holds an Adjunct Professor appointment with the Department of Pathology and Laboratory Medicine. Within the UBC Faculty of Medicine, he’s involved in delivery of both the Bachelor of Medical and Laboratory Sciences (BMLSc) and medical undergraduate (MDUP) programs. Working within a variety of pedological styles has granted him a unique insight into several methods to convey scientific information. He has also recently been appointed as the Associate Director of the BMLSc program and is involved in education and curriculum development.

Since early in my pathology training I developed a major interest in hematological malignancies with a focus on combining routine diagnostic lymphoma practice with translational research. As a result, I did a rotation at BC Cancer Agency’s Lymphoma Tumor Group in 2001. After completing my anatomic pathology residency in 2003 in Lisbon, I come back to BCCA for both Molecular Lymphoma Pathology and Clinical Hematopathology fellowships in Dr. Randy Gascoyne’s lab. This resulted in my PhD thesis entitled: “Biological Predictors of Survival in Lymphoma and Mechanisms underlying Follicular Lymphoma Transformation into Diffuse Large B Cell Lymphoma”.

I joined the BC Cancer pathology staff in 2013 and since then I continue to collaborate in many of Center for Lymphoid Cancer research projects.

My research interests have focused on different lymphoma types with an aim to understanding the mechanisms of lymphoma progression and transformation as well as discovery and development of new prognostic and predictive-target biomarkers. One of the main topics has been several projects related to the tumour microenvironment, specifically studies of the role of immune response in the transformation of follicular lymphoma.

Since 2001, it has been an honor and privilege to meet and work with so many bright colleagues of quite distinct backgrounds both at BCCA and through it with several international lymphoma groups.

Currently and in the coming years, I will certainly continue to integrate BC Cancer both lymphoma pathology diagnostic service and translational research studies at the BCC Centre for Lymphoid Cancer.

I spent my early years in Toronto and attended McGill University to complete my undergraduate degree. I completed medical school at the University of Toronto, and during this time developed an interest in pursuing pathology as a specialty. My love of the west coast combined with my desire to become a pathologist led me to complete my medical residency in Vancouver. During residency I was exposed to renal pathology and decided to explore it further through completing a one year fellowship at Foothills Hospital in Calgary. I returned to Vancouver in 2017 to become a member of the pathology staff at St. Paul’s Hospital.

Dr. Matthews is part of a larger, highly collaborative team which combines the expertise of clinicians, bioinformaticians and basic science researchers to help diagnose patients with rare genetics diseases. When conventional analysis is unable to find the cause of a patient’s disease, the team uses whole genome and exome sequencing to search for what could be a mistake as small as one letter in the 3 billion letters that make up the genome. As a bioinformation, Dr. Matthews’s role is to use computers to help sort through the data and then to apply genetics to prioritize potential causes for each patients. Dr. Matthews has a focus on disease on the X chromosome as well as patients with intellectual disability, metabolic disorders and atypical cerebral palsy.
