Appointment Checklist


To ensure that your appointment is processed in a timely way and to ensure that you receive your pay on the appropriate day, it is imperative that you complete and return the following appointment check list:

If individual’s citizenship is not Canadian, attach a copy of a valid work/study permit. Please note that UBC Payroll now requires that, if the SIN is in the 9-series, a copy of the card and valid work/study permit be submitted for all appointment changes.

Candidates for postdoctoral fellowships should have completed their PhD not more that three years ago or their MD not more than 10 years ago;
Postdoctoral fellows may be appointed for up to 1 year at a time for a maximum of three years.

If individual’s citizenship is not Canadian, please provide a copy of documentation that authorizes individual to work in Canada. Also, please note that UBC Payroll now requires that, if the SIN is in the 9-series, a copy of the card and valid work permit be submitted for all appointment changes. Only cards bearing expiry date are acceptable).

Provide all information / documentation for all new appointments (UAA, UTA, GAA, GRA, GTA)
